The Geriatric Care Management Certification course is designed for those individuals who are seeking to become a Certified Geriatric Care Manager (CGCM), including Nurses, Rehabilitation Services Providers, Long Term Care Professionals, Case Managers, Social Workers and Elder Law / Legal Professionals.

COURSE STRUCTURE & OBJECTIVES

This course is comprised of four fully online self-paced modules, and learning activities in each module include assigned readings, short online videos, knowledge check questions, and case study reviews. The course is designed to be completed in 40 contact hours, and the learning objectives are as follows:
 
  • Explain the foundational concepts associated with Geriatric Care Management.
  • Illustrate the process of conducting a geriatric assessment and brief intervention techniques. 
  • Compare and contrast elements of the psychosocial, functional and environmental assessment.
  • Demonstrate the process to create a care plan that supports the holistic quality of life for a geriatric client. 

Course participants have the option to participate in an online discussion forum hosted by the course instructor on rotating course topics.

  • Conducting an Environmental Assessment
  • Conducing an Economic and Legal Assessment

Module 2:
Ethical, Legal, and Financial Issues in Geriatric Care Management

  • Legal Issues Affecting Senior Citizens
  • Elder Law, Estate, and Life Care Planning 

Module 3:
Developing the Geriatric Life Care Plan & Care Monitoring

  • Using Assessments to Develop Interventions & Write the Care Plan
  • Emergency Preparedness & Disaster Planning / Response 
  • Care Plan Monitoring & Effective Use of Technology 

Module 4:
Working in Different GCM Settings & Private Practice

  • Working in an Elder Law or Life Care Planning Firm
  • Working in Long Term Care Facilities 
  • Adding a GCM Practice to a Home Health Agency
  • Developing a Private Practice Care Management Agency

CEU HOURS EARNED FOR THIS COURSE:

Upon completion of the course participants receive 40 hours of Continuing Education Units based on standards established by the International Accreditors for Continuing Education and Training.

This program has been pre-approved by The International Commission on Health Care Certification.

This program has been pre-approved by the Commission for Case Manager Certification to provide continuing education credit to Certified Case Managers (CCMs).
